Berkshire Sensory Consortium Service
LA, Sensory Services or Schools for the Deaf or Blind/VI /MSI Referral
This form is to be completed by one of the following for support from the Berkshire Sensory Consortium Service (BSCS):
1. A Sensory Education Support Services outside Berkshire who are referring a child or young person (CYP) who is deaf /has a hearing loss, vision or multisensory impairment (VI/ MSI) to the Berkshire Sensory Consortium Service
2. A Local Authority (within or outside Berkshire ) who have been made aware of CYP with sensory impairment (SI )moving in to Berkshire from an other Local Authority and who require peripatetic support from BSCS
3. A specialist School for the Deaf or School for CYP who have a vision impairment who are transitioning into a Berkshire mainstream school, generic special school or college
